Palliative Care Survey

Hello,

 Our capstone group would like to invite you to participate in our research study to assess your familiarity with palliative care, your comfort level with initiating a palliative care referral, and your ability to identify provider barriers to palliative care referrals. Below you will find our SurveyMonkey link that will take you through the consent, pre-test survey, educational PowerPoint, and post-test survey. The link takes about 20 minutes to complete in its entirety. All participants will remain anonymous and all information will be kept confidential.
