Dear Nurse Practitioner

I am seeking advanced practice registered nurses (APRNs) to participate in a research study examining nutrition counseling given to patients in primary care settings.

Providers that practice in a primary care setting are invited to participate. This project is not only evaluating nutrition counseling practices but seeks to understand variables that influence these practices.  

The information you provide will be helpful in understanding more about nutrition counseling practices used among APRNs in the primary care setting. All participants will remain anonymous and all information will be kept confidential.  There will be no information collected that will identify you.  The Survey Monkey® questionnaire site does not collect or release any identifiable information.
