A mission-driven, nonprofit healthcare organization is seeking an experienced Nurse Practitioner to join a growing home-based primary and palliative care program serving patients across North Dakota and Minnesota. Position Overview

• Serve patients primarily in their homes and assisted living facilities (roughly 50/50 split)

• Collaborate closely with an interdisciplinary care team

• Participate in weekly team meetings; otherwise, office visits are infrequent

• Occasional support for inpatient hospice rounding or clinic-based care as the program grows

Schedule & Hours

• Flexible scheduling with provider input (early starts available if preferred)

• Full-time preferred; 30–32 hours/week may be considered

• Minimal evenings/weekends

• Field-based work with independence and structure

Qualifications

Required

• Minimum 3 years of experience as a Nurse Practitioner

• Background in primary care with a geriatric patient population

• Active or ability to obtain licensure in North Dakota and Minnesota

• DEA license required

• Comfort working independently in home and facility-based settings

Preferred

• Home-based care experience

• Palliative care or hospice exposure

• Experience managing medically complex patients

• Physician Assistant candidates also considered

Compensation & Benefits

• Competitive base salary based on experience (typically $110,000–$140,000+)
