Position Description

The provider will cover the full spectrum of care in Family Medicine and participate in regional collaborative partnerships with other Mayo Clinic Health System sites as needed.  The position is within the Department of Family Medicine.

Practice Overview:

  • Currently have 6 providers
  • Currently have 1 full time chiropractor on site M-F
  • Currently have part time social worker on site
  • Have Care Coordination available each week
  • Have RN visits
  • Hypertension, first OB, Medicare Wellness, ACC, etc
  • Have Medication Therapy Management available weekly (Clinic Pharmacy)
  • No Hospital rounding
  • No after-hours call
  • No Nursing home rounding (for APPs)
  • No Emergency Room call / Work
  • Lab and General Imaging Services on site M-F
  • Dietitian on site
  • OB shared care model
  • Higher Somali patient population
  • Located on a city medical campus:
  • This means our providers maneuver continuity of care delicately if patient chooses to stay local (outside records) vs using Mayo partners at a hub site (Mankato, New Prague, etc)
  • Have strong working relationships with these non-Mayo outside agencies.

Collaboration with provider partners on site to help with appropriate care planning for the patients; this provider will be part of Care Team A.  Care Teams:   Team A has 2 physicians and 1 care team RN;  Team B has 3  physicians, 1 NP and 1 care team RN. 

Must support the Mayo Model of Community Care (Team based care model):

  • Patient Portal use
  • Huddles
  • Care Coordination
  • Hand off for various RN and MTM visits
  • Colocation

Must be:

  • Engaged in the community and participate in different community events
  • Attend site / regional required meetings (provider, all staff, etc)
  • Engaged and supportive of quality, patient satisfaction and productivity measures and work to meet targets set by the organization

Will rotate as clinic provider on call with the rest of the Family Medicine providers (1:7) see below on call expectations for detail.  Please note, as the needs of the organization and the Practice change our provider team adjusts and this new provider partner will be expected to adapt as well

Will need to become certified in Anticoagulation within 2 years of hire.

Call: no after hours call, but will rotate in daily clinic call rotation (which means being the go-to' provider for the day, addressing med refills when provider partner is out, acute / same day patients, etc.)

Qualifications:

  • Masters prepared graduate of an accredited school of nursing and Nurse Practitioner. Current Minnesota RN license and certification as a FNP or ANP.
  • Or graduate of an accredited Physician Assistant program with current PA certification by NCCPA and licensure as a PA by the Minnesota Board of Medical Practice.
  • Must be eligible for DEA registration and billing privileges

schedule: Full Time

Schedule Details:

8 a.m. to 5 p.m.; 4 clinic days and 1 administrative day.  This will be based upon current practice needs, ability of nursing support and current clinic hours. We will work with the new NP/ PA on schedule to help establish required contact hours and non-clinic time per patient volume requirements: weekly contact hours reflect 32 to 35 patient contact hours in the clinic and 8 hours non visit care time (administrative time) each week.

Site Description:

St. Peter is rich in historical buildings and besides having about a dozen structures on the National Register of Historic Places, the entire downtown area was also given that designation.  Many of the old buildings downtown still maintain the historical features that have been lost on other buildings.

St. Peter is home to the internationally renowned Gustavus Adolphus College, is the headquarters of the Alumacraft Boat Company, the Minnesota School Board Association, Scholarship America, Hendrickson Organ Factory (a company that makes hand made pipe organs which are shipped around the world) and a state treatment facility.

St. Peter is also proud to host several large celebrations each year.  On July 4th over 10,000 people visit town to take part in the Old Fashioned Independence Day celebration picnic, view the 2 hour parade, attend the drum and bugle corps event and watch the fireworks.  In September the annual Rock Bend Folk Festival takes place in Minnesota Square Park and in October Gustavus hosts the world famous Nobel Conference.  Students and dignitaries from all over the world come to participate in this week long event.
